HEALTH BENEFIT OPEN ENROLLMENT-PLEASE READ ALL INFORMATION BELOW

 

If you are currently enrolled in a CalPERS Health Plan you will soon receive a package in the mail from CalPERS containing the 2020 CalPERS Health Program Guide and Health Benefits Summary Booklets. 

 

If you do not receive a packet, you can request this information by visiting the CalPERS website at https://www.calpers.ca.gov/ or you may contact them at their toll free number 1- (888) 225-7377.

 

IMPORTANT DATES:

 

Open Enrollment Period:               September 9, 2019 – October 4, 2019 – PLEASE DO NOT TURN IN OPEN ENROLLMENT FORMS PRIOR to 9/9/19

 

Effective Date of Change:              January 1, 2020

 

 

WHAT DO YOU NEED TO DO?

 

If you do not wish to make any changes to your health coverage, you do not need to do anything.  Your same health care coverage will automatically roll over for the coming year.

 

If you do wish to enroll or make a change, please follow these steps:

 

  1. Choose and review the Benefit Plan Rate sheet that pertains to you (DSA, GCMMA, General Unit, Non-Represented or POA) along with your 2020 Health Program Guide. Then decide what plan is right for you.

    The following plans are available to Glenn County Employees:

     

    1. PORAC (Note: This plan is only available to certain represented employees: DSA and POA; and some Mid-Management staff)
    2. HMO – Anthem Blue Cross Traditional
    3. HMO – Blue Shield Access+
    4. PERS Care
    5. PERS Select
    6. PERS Choice

       

  3. Complete the Health Benefits Enrollment formhttps://www.countyofglenn.net/sites/default/files/Personnel/Benefits/health-benefits-enrollment-form.pdf.  You will need to complete boxes: 1 through 7, 9 through 13 (Permitting Event date will be the date you fill out the form, within the Open Enrollment period only), 14, 15 (Please fill out for yourself and all dependents, see Relationship Codes at bottom of box. And remember to check the Action box that pertains to your enrollment), 16 OR 17, sign and date boxes 18 and 19. The Personnel Department will fill out Section H on the back of the form.

     

    NOTE: In Box 15 – you must list ALL dependents you wish to have enrolled in the plan.  If you do not list a dependent that you currently have covered, we will assume that you have dropped that dependent. 

     

  4. If adding NEW dependents, you will need to provide a Birth Certificate or Marriage License (as applicable). You must also provide the dependents social security number.

     

  5. Submit all documents to the Personnel Department NO LATER THAN 5:00 p.m., Friday, October 4, 2019.  Any documents received after this date will not be considered for the Open Enrollment Period.

     

  • Documents may be submitted via fax at 934-6452, email, mail or in person at 525 W. Sycamore Street, Willows, CA 95988.

 

Please note – if you currently receive the “Cash In–Lieu” benefit and wish to continue this benefit – you must re-certify your proof of other health coverage and fill out the enrollment form - https://www.countyofglenn.net/sites/default/files/Personnel/GCPER16-CashIn-LieuofHealthCov2014.pdf. Failure to submit proof of other health coverage along with the enrollment form by Friday, October 4, 2019 will result in loss of benefit. 

 

Ancillary Benefits - Enrollment options for Dental, Vision, and Life will be available during the ancillary benefit open enrollment period in November. Details will be available in October.
